Types of Practice Coaching

1. Qualis Health Practice Coaching

Qualis Health is a non-profit quality improvement organization that has worked nationwide with foundations, health plans, and state governments to help primary care sites become high performing patient centered medical homes. Qualis Health’s experienced consulting team brings deep knowledge of PCMH content, clinic operations, quality improvement methods, and health information technology. Qualis Health’s practice coaching services are based on eight change concepts for practice transformation.

Qualis Health will use their proven evidence-based model to provide practices with a 12-month engagement providing full spectrum support. This support includes a baseline PCMH assessment (to determine strengths and opportunities for advancement), development of a practice transformation plan created in partnership with the practice, and tailored technical assistance through on-site and remote practice coaching. 2. Coleman Associates Rapid Dramatic Performance Improvement

Coleman Associates structures their approach to move practices forward in finding new ways to improve productivity (i.e., capacity to see patients), increase patient access, improve patient flow, reduce patient waiting time, develop teamwork and optimize care teams, improve staff satisfaction, develop PCMH practices, and optimize the use of technology. A team of 3-5 staff from Coleman Associates conducts an initial 4-day visit at the practice site, followed by eight weeks of distance coaching, a 2-day site visit midway through the distance coaching series, and a 1-day capstone visit by the Coleman Associates project coach.

The Coleman Associates team works closely with the clinic management team during site visits. The team meets with managers and key staff members twice daily in “huddles” to share observations and information, get feedback, discuss recommendations, and agree on an implementation plan for the following day, as well as coordinate overall efforts related to the practice coaching engagement. Key outcomes from PCMH practice coaching engagements may include:

Engaged Leadership: Providing visible and sustained leadership committed to supporting a culture of quality improvement. Providing resources for teams to implement appropriate practice change.
Empanelment: Linking patients to a provider or care team.
Organized, Evidence-Based Care: Utilizing evidence-based care processes throughout patient care in a standardized manner and performing chronic and preventive services at every visit.
Enhanced Access: Structuring clinic hours to reflect appointment demand, and offering same day appointments. Providing resources for teams to implement appropriate practice change.
Team-Based Care: Care teams within clinics work together to provide quality care to all patients.
Patient-Centered Care: Patients are encouraged to take an active role in their care and treatment.
Care Coordination: Integrating behavioral health, specialty care, and primary care services and partnering with community resources.
Data-Driven Quality Improvement: Training staff in quality improvement methods, using data to track progress of improvement projects, and altering projects as necessary.
